There are no copays or deductibles to worry about. Plus, no dealing with denial of claims, pre-approvals, waiting periods, or annual maximums. The membership plan is not (i) insurance, (ii) a qualified health plan under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to saved services. 

Plan membership cannot be combined with current dental insurance plans. This is a dental savings plan which offers savings to patients who wish to join it and who do not have dental insurance. Cannot be used for services treating injuries covered under workers compensation or medical insurance.

Plan savings do not apply to procedures that are not offered at our practice. Plan does not apply to specialist referrals, hospital, or any other office. Not applicable towards purchase of in-office products.

The membership plan runs for 12 months and fee is due upon joining. Plan is effective on the day payment is received. The membership plan includes a 30-day money back guarantee. You can receive a full refund up to 30 days after purchase if no services or savings have been used.

Once the membership plan has been used or the 30-day window has lapsed, there are no refunds. Membership benefits are not transferable, have no cash value and may not be redeemed for cash. No refunds will be given in the event the member terminates the plan prior to the end of the plan year.

Is it the sole responsibility of the patient to maximize benefits within the 12 month plan period. The member is not entitled to a refund if benefits are not used within the 12 month period.If recommended treatment needs are not completed within the 12 month plan period, discounted  rates do not apply.

The annual membership fee must be paid in full in order to receive the services and savings included in the plan. The plan is for individual use only. It is not a group benefits plan.

Total payment amount for discounted services is due at the time services are provided. If full payment is not received at the time of service, fee discount will be void.

Missed appointment fees are ineligible for the membership discount.

Membership cost and plan savings may not be combined with any other offers or savings.

Plan rates may be reviewed and adjusted on an annual basis.

The dental practice retains the right to interpret any program stipulations.
